City Fresh, in partnership with ProducePerks Midwest, is pleased to offer the Free Shares program for the third consecutive year.
In 2024 we will be enrolling 91 eligible families for up to 16 weeks of free Family Shares, with a free home delivery option for those who face additional barriers.

Qualifications
1. U.S. citizen or national, or meet one of these citizenship exceptions;
AND
2. Have a minor child in the household, or are pregnant;
AND
3. Have not enrolled in any Fruit and Vegetable Coupon program in the last 6 months;
AND
4. Household income is at/below:

It is VERY IMPORTANT to use a valid e-mail address in this form. There will be multiple steps and digital forms required in order to complete your registration. These will all be communicated via e-mail over the course of a few weeks. If you do not complete all the steps in the follow up e-mails, you will NOT be able to participate in this program.
